Summary: Responsible for managing patient populations with acute, chronic illness and/or multiple co-morbidities, identified as requiring additional transition of care support following visit to the Emergency Department setting. Works in coordination with multiple PCP's and/or community providers and serves as the central link to an identified population of patients. Through skilled assessment, education, collaboration and coordination of healthcare and community resources, the ED Care Navigator assist patients to gain self-efficacy/management skills, achieve optimum functional health status and quality of life. Utilizes professional nursing, and critical thinking skills to assess patient and family needs for clinical appropriateness for the setting and establishes plans for effective management of the continuum of care. Collaborates with all healthcare disciplines and coordinates patient care for smooth, safe, quality, safe transitions. Performs functions related to Patient Advocacy; Utilization Review; Resource Management; Continuum of Care and Management (Discharge planning). Assists with the determination of patient level of care and assures appropriate level of care is correctly assigned to patients. Performs functions to prevent financial loss to the hospital while ensuring high quality of care for all patients.

Education/Experience: Must be a Registered Nurse with BSN preferred. Hold a current Rhode Island license. Minimum of 5 years clinical experience in an acute care medical/surgical setting. Computer databases management experience. Case Management experience preferred. Experience in ED as a registered nurse or case manager preferred.
